• List of medications you are currently taking
• List of allergies
• Insurance information and photo ID
• List of past medical evaluations and/or records
• Any x-ray or MRI films/reports you may have

Payment and Insurance

The Advanced Neurology Institute of Nevada is a provider for Medicare and most major insurance plans. We provide insurance billing for primary and secondary insurance. Anything not covered by insurance will be your responsibility. Given advance notice, our office staff can help you with a flexible payment plan.

We request payment at the time of your appointment for services that are not covered by insurance. Your insurance company may require you to pay a co-payment. This is due at the time of your appointment. Credit cards are welcome.

Hospital Privileges

Dr. Broeske will see patients in consultation at Sunrise Hospital and Desert Springs Hospital, if he has seen them previously in an office setting.
